Medication Errors and Near Misses

This form is used to report Medication Errors and Near Misses.

Near misses are times when a medication error is almost made. Reporting these cases helps us to identify system design problems which we can fix, or educational issues which could be of benefit to all of our providers.

The Medical Control Authority is dedicated to fostering a non-punitive environment, focused on process improvement and education. If EMS providers self-report medication errors when they occur, we will not suspend or revoke privileges to function under normal circumstances.

We would much rather hear from the person who made an error, or had a near miss, than to hear from others about an unreported problem.

Our overarching goal is to find ways to make this the safest EMS system with as few medication errors as is possible. Your help is greatly appreciated!
