CE Requests MUST be submitted at least 10 days prior to desired course date.

Incomplete or innacurate forms will be returned as not approved.

Course must recieve CE Sponsor approval from West Michigan Regional Medical Consortium before being conducted.

If you do not recieve approval notification within 3 business days, please contact the Program Sponsor Representative Chad Lawton, or the Program Director Scott Wilkinson.
