Please use the following forms to request follow-up information on a patient if:
- You are currently a privileged EMS provider and associated with a life support agency that falls under Kent County Emergency Medical Services
- You must have provided direct prehospital patient care for the patient whose information is being requested
- The patient has to have been transported to a healthcare system within Kent County (Corewell Health, Trinity Health GR, U of M West)
Any patient follow-up information must be sent to an EMS agency-specific email address. Please do not provide personal email addresses in your requests.
